ABSTRACT
Autoimmune cerebellar ataxia (ACA) is an immune-mediated cerebellar syndrome that is often associated with specific autoantibodies. We report a case of a 61-year-old man with small cell lung cancer who developed subacute progressive cerebellar ataxia after durvalumab therapy. Anti-P/Q-type voltage-gated calcium channel (VGCC) and anti-metabotropic glutamate receptor 5 (mGluR5) antibodies were detected. The patient improved substantially with early immunotherapy and the discontinuation of immune checkpoint inhibitor (ICIs) therapy. In this case, anti-VGCC antibodies may have contributed to the pathophysiology. However, anti-mGluR5 antibodies were also detected. The relationship between ACA and ICIs therapy remains unclear, and additional cases are needed.
